Blue Ridge Hospice has been the exclusive provider of licensed hospice care to people in a five-county area since 1981. A private, non-profit, community-based organization, Blue Ridge Hospice is licensed by the Commonwealth of Virginia and certified by Medicare/Medicaid to provide hospice and home heath services. We are accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO). Included in our services is a free-standing, 8-bed residential center.Our location near the Blue Ridge Mountains in Virginia offers a wide range of living choices, easy access to Washington, D.C., and all the amenities of the Northern Virginia Area including first-rate schools, cultural activities, and the pleasures of country and/or suburban environments.

RN Case Managers , Home Care – Full Time Positions (1) Front Royal & (2)Shenandoah County
• Position involves travel, within a specified local territory, to patient homes to provide direct patient care and education to Hospice patients and their caregivers
• Current RN state license required
• Position requires minimum of 2 years current acute care experience
• Previous hospice experienced preferred
• Must have excellent assessment, critical thinking and problem solving skills
• Must be self-directed, motivated and highly organized
• Ability to work within an interdisciplinary team setting
• Must be familiar with general use and functions of the computer
Certified Nursing Assistant (CNA) - - Home Care – Full Time Position
• Position involves travel, within a specified local territory, to patient homes to provide direct patient care and education to Hospice patients and their caregivers-Front Royal/Warren County and surrounding areas
• Current CNA state license required
• Under the direction and ongoing supervision of the registered nurse, provides essential personal care assistance to the patient. May involve assistance with bathing, ambulatory needs, exercises, assists with self-administered medication, assists with eating
• Takes and records patient’s temperature, blood pressure, pulse and respirations.
• Demonstrates visit closure and relationship termination skills. Maintains professional boundaries
• Articulates, identifies, and reports signs and symptoms of patient abuse and/or neglect, suicidality, or violence
• Reports any changes in the patient's condition or family situation immediately to the Primary Nurse, Director of Nurses or office nurse
• Attends at least twelve (12) hours of continuing education programs or approved audiovisuals per calendar year
• Previous hospice experienced preferred
• Must have excellent assessment and problem solving skills
• Ability to work within an interdisciplinary team setting
• Must be familiar with general use and functions of the computer
Assessment Nurse - Full Time Position in Winchester, Virginia
Hospice and Palliative Medicine Physician – Full Time Position
• Provide inpatient and outpatient palliative consults
• Provide hospice homecare patient visits
• Participate in interdisciplinary hospice home care team meetings
• Manage Palliative patients’ pain and non-pain symptoms with medication and other treatment.
• Evaluation for hospice certification of terminal prognosis of patients, quality of initial plans of care and comprehensive plans of care, revocations, re-certification of terminal prognosis, quality of pain an symptom management
• Ensure accurate documentation of patient care to support reimbursement for services provided.
• Adhere to the practice of confidentiality regarding patients, families, staff and the organization.
• Participate in on-call rotation as needed
• Doctor of Medicine or Osteopathy licensed to practice medicine in the state of Virginia.
• Board certification in a primary care specialty, Board certified in Hospice and Palliative care preferred.
• Current DEA license
• Proven track record with interdisciplinary teams, effectively establishing goals of care and building collaborative physician/patient/family relationships in addition to a comprehensive knowledge of hospice and palliative care, pain management and medical ethics preferred
• Previous hospice and/or palliative care experience preferred
